Speech reabilitation after near total laryngectomy.

Abstract:
Between April 1990 and April 1991 nine patients, all mate, ages from 53 to 78 years old average of 61 years, with the diagnoses of squamous cell carcinoma of the larynx in 5 cases and of the pyriform signs in 4 cases, were submitted to a near total laryngectomy with simultaneous unilateral neck dissection. A mucosa flap from the hypopharynx were used in two cases to make the trachealesophageal shunt. Post-operative radiation was indicated and done in 4 patients. We had 3 cases of cervical infection and 3 cases of fistula after the surgery but all cases had a good response with conservative care only. All patients but one acquired between the 1401 and 901h post-operative day Speech whose quality was superior than the esophageal Speech and the eletronic larynx. Slight aspiration was observed in two patients. Two patients died of distant recurrence 5 and 8 months after the surgery. The other seven patients have not shown signs of recurrence with a follow-up between 2 and 14 months. We conclude that the near total laryngectomy provide a good quality of vocal rehabilitation with incidence of sucess, it is compatible with post-operative radiation and retains the patient's capacity of oral nutrition.
