Conservation Surgery in the Treatment of Laryngeal Squamous Cell Carcinoma.

Abstract:
The surgical treatment of laryngeal squamous cell carcinoma is realized most of times by-total laryngectomy, with or without neck dissection, wich take high mutilation degrees. The T2 and T3 tumors may be treated by conservating procedures, depending on tumoral extension. - The authors present four cases of laryngeal squamous cell carcinoma submitted to hemilaryngectomy in the period of 1989 to 1992, at the head and neck services from Hospital Prof. Edmundo Vasconcelos (São Paulo) and Hospital e Maternidade Celso Pierro of Pontifícia Universidade Católica de Campinas. The staging were T2N0M0 in two patients, T3N1M0 and T3N0M0 in the two others. There were two supraglottic and two glottic lesions. The larynx was reconstructed in all cases with platisma coli miocutaneous flap, which didn't show necrosis in any of them. Three patients evoluted with acceptable fonation and, in one case (T3), it was not possible to take off tracheostomy tube. There was one local recurrence, wich evoluted to death one year after treatment. Another patient presented local recurrence after two years, being necessary total laryngectomy. Two patients (one patient T2N0M0 and other T3N0M0) kept assymptomatic at the fourth year after surgery. These way, hemilaryngecto promote local control and preservation of laryngeal function in selected patients. The use of platisma coli miocutaneous flap is a good technique for laryngeal reconstruction, because it presents good viability and thickness what provides enough airway.
