Oesophageal Perforation by Suppurative Thyroditis: case Report.

Abstract:
The authors report a case of acute suppurative thyroiditis with oesophageal perforation treated at the Head a Neck Service - Prof. Edmundo Vasconcelos (Gastroclínica) Hospital, São Paulo, Brazil. The patient was a 71 years old white woman who had beern complaint of left, supraclavicular fossa mass, disphagia and dyspneia during the last 6 months, when she loocked our Service. The cintilographie and ultrasonographic exams showed a cold and solid mass occupying thyroid gland, mainly the left lobe. The radiological exam with barium reveal oesophageal stenosis and displacement of the trachea a cervical oesophagus to the right side. With these result was made a diagnostic hypothesis of malignant neoplasia of thyroid gland. The patient was submitted to surgery when it was found an enlarged gland with an abscess its left lobe, which caused fistula to the lateral wall of t cervical oesophagus. Therefore, it was performed to thyroidectomy, suture of the fistula and nasogastric tube. It followed that acute suppurative thyroiditis is a rare disease, oesophageal perforation induced by thyroiditis wasn't find among searched literature and the treatment is done with antibiotics and surgical drainage, at least.
