Abstract: The sphenoid sinus is a relative inaccessible cavity avoiding an optimal physical examínation. In addition, the symptoms of the sphenoid sinus disease are typically noncharacteristic and nonlocalizing. The authors describe a series of four patients with sphenoid sinus involvement and its difficulties in the clinical diagnosis. One patient was a young woman with a frontal sinusitis complicated with subdural empiema that one year later presented with sphenoid sinus opacification at the CT-scan. The second one was an old diabetic woman presented with cavernous sinus tromboses complicating a sphenoid sinusites. The third was a teenager with an íntracranial neoplasm showing opacification of the sphenoid sinus following neurosurgery. The last one was a child with cavernous sinus trommboses and sphenoid sinus sinusites. The routine use of computed tomography on these patiertts evaluation and eventually magnetic ressonance imaging assist in the detection of the sphenoid sinus involvement.
