Abstract: Angiofibroma is a rate vascular benign tumor; that grows slowly and silent. In advanced cases, an extensive destruction of the naso-sinusal estructures, and accometiment of nasopharynx, esphenopalatine and infratemporal fossa often can be seen. An intracranial extension can occur, displacing the dura, without invasion. On recurrent tumors, Me local alterations induced for the treatment could promove a real dura invasion. Between 1967 and 1994, 123 cases of juvenile angiofibroma were treated at the ENT Department of the University of São Paulo. Only 3 cases presented a dura invasion, all of then witlz cavernous sinus accometiment. Two of them were recurrent tumor, but one had no previous treatment. These patients were treated by parcial intracranial resection, following by radiation and hormone therapy, with success. The objective of this paper is to report our experience with this unusual presentation, and discuss the diagnosis and the treatment in this cases.
