Abstract: Cor pulmonale syndrome due to upper airway obstruction is characterized by stridor, sleepiness, adenotonsillar enlargement, cardiomegaly, right ventricular-hypertrophy and signs of heart failure. The authors call attention to early diagnosis of the disease and present a case of patient with adenotonsillar enlargement and cor pulmonale who had been submited to adenoidectomy without any clinical improvement of his cardio-pulmonary conditions. Regression of cor pulmonale and heart failure were obtained only after tonsillectomy.
