Abstract: Pulsatile tinnitus generally originates from vascular structures and results from turbulent blood flow or luminal alterations involving some vessels. A 38 year-old, female patient, during an acute episode of chronic otitis media in the right ear complanied of mild deafness, otorrhea and pulsatile tinnitus. The latest was the most bothersome symptom and many doctors had already told her in previous visits that it was just one more symptom of the infection. Therefore, the MRA revealed an extensive internal carotid aneurysm, near the emergence of the ophthalmic artery. The patient undergone neurosurgery and the tinnitus was gone. The authors emphasize the investigation of tinnitus characteristics, once some of the etiologies of tinnitus have good results with the presently available treatments.
