Abstract: The authors present an unfrequent case of Wegener granulomatosis in which the first symptom was hearing loss on the left side, followed by peripheral facial paralysis on the same side. In a short time, there was involvement of the nose kidney and lungs. The examination of tissue obtained either from the mastoid, during surgical facial nerve decompression, or from lesions in the nasal turbinates did not make the diagnosis. Histologic examination of two nodes obtained from the right lung has resulted in a Wegener granulomatosis compatible tissue. After immunosuppressor treatment, the lesions were controlled and the patient clinically improved. This and the follow-up of the patient, during the latest 4,5 years, presenting no recurrence of the lesions, confirm the diagnosis.
