Abstract: We present a case report of orbital floor and right orbit median wall fracture caused by car crash, with injury of orbital cleft, imprisonment of the lateral straight rectum muscle between the osseous fragments of the floor and herniation of extra conal fat inside the right maxillary cavity. After 30 days of clinical observation, there was no voluntary solution and a surgery was indicated. For this case, we introduce a new treatment option to the orbital floor correction that behaves like this case, through the transmaxillary osteoplastic technic, with the use of self graft free of septal cartilage. The technic has proved to be a pratical, effective and not much traumatic procedure.
