TÍTULO: RECORRÊNCIA DA POLIPOSE NASAL EM CRIANÇAS E ADOLESCENTES COM FIBROSE CÍSTICA DURANTE SEGUIMENTO DE 5 ANOS

Introduction: Cystic fibrosis is frequently associated to nasal polyposis (NP).

Aims: To study the incidence and recurrence of NP in children and adolescents with cystic fibrosis, during a five-year period of follow-up.

Methods: Twenty-three cystic fibrosis patients were submitted to routinely nasal endoscopy at every six months, during the period from January 2005 to January 2010. Clinical symptoms, sweat chloride level and genotype were described. Presence and degree of NP were studied by endoscopy. Polyposis was treated with topical corticosteroid for six to 12 months, recurrence was described.

Results: At first endoscopy, NP was found in 39.1% of the patients (five bilateral, four unilateral) with no association to sweat chloride level, genotype, clinical signs of severity of CF or nasal symptoms. During the five years, 16 patients (69,5%) experienced at least one event of polyposis, the youngest patient diagnosed NP at age of 32 months. At last endoscopy, three patients had NP, all as recurrency. Recurrence occurred in 10 patients (62,5%), two patients had more than one recurrency. Four patients died during the period, 2 suffered endoscopic surgery, and one developed carcinoma of the nasopharynx.

Conclusion: The study showed a high incidence of NP, even in the absence of clinical nasal symptoms.  Recurrence rate is elevated, but clinical treatment still improves NP, surgery being necessary in a few cases. Routinely endoscopic examinations are highly recommended.
